Outpatient RN Case Manager

Schedule
Day shift, 5 x 8-hour shifts

Requirements
- RN license required
- Minimum of 1 year prior RN experience
- BLS certification required
- Experience with geriatric/dementia populations
- Familiarity with blood pressure and blood sugar monitoring

Roles & Responsibilities
- Manage chronic disease case management and care coordination.
- Monitor medical conditions and communicate with interdisciplinary teams.
- Provide caregiver education and support.
- Collaborate with healthcare providers to optimize patient outcomes.

Compliance / Notes
- Reliable transportation required
- Two references required covering one year within the last three years
- Orientation duration: 8 hours
- Basic color scrubs permitted
