In this rewarding role, you will evaluate patients in both inpatient and outpatient settings, creating personalized treatment plans that address their unique needs. You will administer physical therapy interventions that focus on mobility, strength, balance, and overall function. Collaboration is key; you will work alongside nurses, physicians, case managers, and various rehabilitation professionals to optimize patient outcomes while educating individuals and their families on therapeutic goals, discharge planning, and necessary home exercises. Documenting evaluations and treatments in accordance with facility and regulatory guidelines, you will actively participate in interdisciplinary rounds and conferences to ensure cohesive care for your patients.

To join us, you will need a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program. State licensure as a Physical Therapist is a must, along with CPR certification which is preferred. Specialized certifications, such as those in neurology or geriatrics, are welcome but not required. The right candidate will possess prior experience, the capability to treat individuals with complex medical and mobility challenges, and strong communication and teamwork skills. Comfort with digital charting and clinical documentation systems is essential.
