Hospice Community Education & Partnerships Lead

Heart'n Soul Hospice

Heart’n Soul Hospice is seeking a Community Education Representative in Tacoma, WA to grow hospice referrals and strengthen community resources. You will work with physicians, hospitals, and senior living facilities to promote Heart’n Soul Hospice House and its services.

The role emphasizes territory planning, relationship-building, and proactive outreach, with a strong focus on Medicare Hospice Benefit knowledge and autonomous time management.
