Location: Assisted & Independent Living Communities
Schedule: Full-Time (minimum of 30 visits/week) | Monday-Friday
Compensation: Per-Visit to Salary
Total Care Therapy (TCT) is a therapist-owned and operated company providing Physical, Occupational, and Speech Therapy services in assisted and independent living settings. Our mission is simple: empower individuals to regain independence through compassionate, high-quality care.

Position OverviewWe are seeking a passionate and patient-centered Physical Therapist (PT) to join our growing team. In this role, you will deliver high-quality therapy services to residents in assisted living environments, helping them restore mobility, function, and confidence.
Key Responsibilities
- Provide treatment visits, 30-day reassessments, re-certifications, and discharge evaluations
- Develop and implement individualized treatment plans with measurable short- and long-term goals
- Supervise and mentor Physical Therapist Assistants (PTAs)
- Manage a full caseload (28-32 visits per week for full-time positions)
- Maintain accurate documentation using an electronic medical record system
- Recommend and assist in fabrication of adaptive devices as needed
- Educate patients, families, and caregivers on therapy techniques and equipment use
- Collaborate with interdisciplinary healthcare teams to ensure coordinated, high-quality care
- Degree in Physical Therapy from an accredited program
- Active state Physical Therapist license
- CPR/BLS certification
- Valid driver’s license and auto insurance
- Strong knowledge of medical terminology and therapy best practices
- Excellent communication and interpersonal skills
- Compassionate, empathetic approach to patient care
Preferred:
- Geriatric experience
- Experience with care planning, discharge planning, or inpatient settings
