Optum Home & Community Care is hiring a National On Call Advanced Practice Clinician (APC) to provide telehealth and telephonic care to patients, caregivers and care facilities. This remote, per diem role offers 24/7 coverage including holidays and requires availability for 32+ hours per month with additional shifts as needed. The APC will perform comprehensive assessments, collaborate with physicians, and document in CMS-compliant formats while using secure video/telephonic platforms and EMR #J-18808-Ljbffr Registered Nurse
