Job Summary:

Provide comprehensive case management services to hospitalized patients, coordinating care plans and ensuring optimal clinical and discharge outcomes. This role requires delivering patient-centered care through interdisciplinary collaboration and adherence to regulatory standards.

Responsibilities:

• 
  Evaluate patient needs and develop individualized care plans.
  
  
• 
  Collaborate with healthcare team members to coordinate services and resources.
  
  
• 
  Monitor patient progress and modify care plans accordingly.
  
  
• 
  Document clinical assessments and interventions accurately in Meditech, Midas, and Interqual systems.
  
  
• 
  Educate patients and families regarding treatment options, discharge instructions, and follow-up care.
  
  
• 
  Ensure compliance with hospital policies, accreditation standards, and regulatory requirements.
  
  

Qualifications:

• 
  Valid Registered Nurse (RN) license with active state licensure.
  
  
• 
  Minimum of 2 years of acute (hospital) case management experience.
  
  
• 
  Proficiency in Meditech, Midas, and Interqual systems.
  
  
• 
  Strong communication and clinical assessment skills.
  
  
• 
  Ability to work independently and as part of a multidisciplinary team.
  
  
• 
  Available to work every other weekend and adhere to scheduled hours.
