Position Overview

Angina Cardiology & Family Health Clinic in Plano, Texas, is seeking a dedicated and compassionate Family Nurse Practitioner to join our team on a full-time basis. In this role, you will leverage your expertise to provide comprehensive, patient-centered care to families and individuals in the community. Work Environment

This position is available onsite at our outpatient office in Plano, Texas. You will work in a collaborative and dynamic environment, supported by a team of healthcare professionals who share a commitment to community health. Your average workweek will total around 40 hours, with typical hours from 9:00 a.m. to 6:00 p.m. Some flexibility is required, as occasional evening clinics may extend to 8:00 p.m., and participation in community outreach on weekends may be necessary.

Key Responsibilities

As a Family Nurse Practitioner, you will be responsible for:

• Conducting thorough health assessments, including physical exams and screenings for both individuals and families, to identify health risks and needs.
• Diagnosing and managing acute and chronic illnesses, creating tailored care plans that include preventive measures and health education.
• Collaborating closely with community organizations to implement innovative health education programs aimed at promoting wellness and chronic disease management.
• Providing culturally competent care, ensuring that you advocate fiercely for patients' rights and facilitate equitable access to healthcare services.
• Managing referrals and coordinating follow-up care to ensure seamless patient experiences and continuity of care.
• Maintaining accurate, detailed medical records in compliance with HIPAA regulations and organizational standards.
• Engaging in community outreach events and health fairs by providing essential health screenings and education to inform and empower the community.
• Staying abreast of current healthcare trends and public health policies relevant to the community to continually improve patient care and outcomes.

You will primarily serve a diverse patient population within the Plano community, focusing on empowering families and individuals from various cultural backgrounds. Your role will involve advocating for and addressing the unique healthcare needs of underserved populations, ensuring that every patient receives equitable, high-quality care regardless of their background.

Qualifications

To be considered for this position, you must possess the following:

• A current certification as a Nurse Practitioner.
• An active nursing license in the state of Texas.
• A Bachelor’s degree in nursing; a Master’s degree in nursing is strongly preferred.
• A minimum of 3-5 years of clinical experience, particularly in community health or public health settings.
• Excellent interpersonal and communication skills, with demonstrated ability to connect with diverse patient populations.
• A strong commitment to social justice and health equity, aimed at improving healthcare access for vulnerable communities.

Preferred Experience

Ideal candidates will have experience in:

• Providing comprehensive primary care in an outpatient setting.

Compensation and Benefits

This position offers a competitive salary range of $180,000 to $200,000 per year, commensurate with your experience and qualifications.
