The fully integrated primary care office features on-site labs, specialty services, and embedded support including behavioral health and pharmacy, with exceptional mentorship from an on-site Regional Medical Director.

Advanced Practitioners - Primary Care

Job Details:

• Full-scope outpatient primary care serving adult medicine patients (18+)
• Open to Nurse Practitioners (APRN) and Physician Assistants (PA)
• Full-time schedule, Tuesday through Friday with flexible scheduling options available
• No call required
• Hands-on procedures including injections and biopsies
• Work within a fully integrated primary care office with on-site labs and specialty services
• Collaborate with embedded behavioral health and pharmacy support staff
• Perform routine physical examinations and formulate medical, educational, and psychosocial plans
• Develop plans for further evaluation of health needs including referrals to specialists and other health team members
• Provide appropriate treatment within scope of practice per established protocols
• Unrestricted State of Connecticut Medical License required (or eligible)
• APRN (Family, Adult/Geri Nurse Practitioner) or PA training required
• Minimum 2+ years of experience in a primary care or relevant fast-paced outpatient setting required
• Utilize EPIC for streamlined clinical documentation and continuity of care
