LVN / LPN - Ambulatory Care/Clinic/Outpatient

Job Description

Job Summary:
The Licensed Practical Nurse (LVN/LPN) in this outpatient clinic setting provides medication dispensing and patient care services within a substance use disorder treatment program. The role involves ensuring medication safety, maintaining accurate records, and collaborating with the healthcare team to support patient recovery.

Responsibilities:

• Administer prescribed medications to patients following clinical protocols and regulatory guidelines.
• Operate automated dispensing systems, calibrate pumps, and maintain dispensing equipment.
• Document medication administration, patient assessments, and treatment reports accurately and timely.
• Ensure secure storage and distribution of controlled substances in compliance with federal and state regulations.
• Manage medication inventory, including ordering, receiving, and stocking supplies as per DEA and licensing standards.
• Perform daily patient assessments and provide support as directed by supervising staff.

Qualifications:

• Hold an active Illinois (IL) LPN license with Nursys verification.
• Demonstrate at least 1 year of medication administration experience.
• Have prior experience working with substance use disorder patients (highly preferred).
• Maintain current BLS certification.
• Provide two supervisory reference forms completed within the past two years.
• Possess skills related to medication management and patient assessment relevant to outpatient care.
