Physical Therapist – Rehabilitation

Schedule: Shift: 8 hours

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40 hours per week

Requirements:

• Current State License as a Physical Therapist
• Proof of current CPR certification
• Current driver's license and reliable transportation
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities:

• Perform clinical assessments to identify patient needs and determine appropriateness for therapy.
• Document and prepare/update care plans, progress notes, and clinical records on an ongoing basis.
• Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders.
• Instruct patients and families on safe use of assistive devices and proper transfer techniques.
• Provide patient and family education regarding therapy procedures based on individual needs.
• Collaborate with an interdisciplinary team to ensure comprehensive patient care.

Compliance / Notes:

• Must meet mandatory continuing education requirements.
• Apply infection control and universal precautions during patient care activities.
