Work Environment\n

This role offers the flexibility of a remote work setting while being an integral part of a collaborative hospice center team. Our social workers engage with clients via telehealth technologies and remote communication platforms, allowing for a personalized and responsive approach to care. The work hours are established from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours required depending on client needs.

Key Responsibilities\n

• Conduct comprehensive assessments that evaluate the psychosocial needs of seniors, incorporating emotional, mental, and social factors.

• Develop and implement personalized care plans in collaboration with clients and their families, addressing specific goals and preferences.

• Provide counseling and emotional support to seniors and their families during transitions and challenges, promoting resilience and coping strategies.
\n
• Advocate for client rights and facilitate access to essential community resources, ensuring clients receive the required support.
\n
• Engage with multidisciplinary teams—comprising nurses, caregivers, and healthcare professionals—to coordinate care that enhances overall client well-being.

• Perform regular follow-ups to monitor clients' well-being and living conditions, addressing any emerging concerns or safety risks.
\n
• Facilitate constructive communication and conflict resolutions within families involved in the care of senior clients.
\n
• Educate clients and families about community resources, support groups, and programs that can improve social engagement and quality of life.

• Maintain meticulous documentation of client interactions, assessments, care plans, and progress without compromising compliance with ethical standards.

• Stay updated on best practices in social work, as well as relevant policies and regulations.

Patient Population / Client Focus\n

This position will primarily focus on senior clients, many of whom are facing complex health and social challenges. Your expertise in geriatric social work will directly contribute to improving their emotional well-being and quality of life, as you help them navigate their care journey during hospice provision.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid licensure as a Clinical Social Worker (LCSW) in Texas.

• At least two years of relevant experience in a social work role, ideally in a community center or geriatric environment.

• Strong understanding of social work principles, aging-related issues, and community resources.
\n
• Demonstrated skills in assessment, counseling, and problem-solving.

• Able to empathize with clients, showing compassion and establishing rapport easily.

• Exceptional communication and interpersonal skills for effective teamwork.
