Location: Phoenix West Valley, AZ – Territory west of I-17
Setting: Home Health
Employment Type: Travel / Contract
Start: ASAP
Weekly Pay: $2,359–$2,622 per week based on 36–40 hours
Mileage Reimbursement: $0.76/mile

About the Opportunity

We are seeking an experienced Physical Therapist (PT) for an ASAP Home Health opportunity serving the West Valley of Phoenix, Arizona .

This is an excellent opportunity for a confident Home Health therapist who enjoys working independently, building relationships with patients and families, and helping patients improve their mobility and independence in the comfort of their homes.

Responsibilities

• Complete in-home Physical Therapy evaluations and reassessments.

• Develop individualized plans of care based on each patient's functional needs and goals.

• Provide therapeutic exercise, gait training, balance, mobility, and functional training.

• Educate patients and caregivers on home exercise programs, safety, and fall prevention.

• Coordinate care with physicians, nursing, therapy, and other members of the interdisciplinary team.

• Complete accurate and timely clinical documentation.

• Manage an assigned Home Health territory and patient caseload efficiently.

Qualifications

• Graduate of an accredited Physical Therapy program.

• Active Arizona Physical Therapist license in good standing.

• Previous Home Health experience required.

• Current BLS/CPR certification.

• Valid driver's license and reliable transportation.

• Strong time-management, communication, documentation, and independent clinical decision-making skills.
