Field Nurse Practitioner - (South Bay Region)

Job Description Summary Responsible for managing patient care and treatment through in‑home clinical visits, in close collaboration with the physician and interdisciplinary care team. What You’ll Do Conduct in‑home health assessments and focused physical exams Manage acute and post‑acute conditions for patients transitioning from hospital, ER, or nursing facilities Develop, implement, and document individualized care plans Order, interpret, and evaluate diagnostic tests Prescribe medications and treatments within scope of practice Collaborate closely with physicians, care managers, and interdisciplinary team members Educate patients and families to support recovery, safety, and long‑term health at home Maintain timely, accurate clinical documentation Qualifications Master’s degree in Nursing (MSN) with Adult/Geriatric Nurse Practitioner certification Minimum 1 year of Nurse Practitioner clinical experience (home‑based or transitional care experience a plus) Active, unrestricted RN and NP license in the applicable state Current TB test or willingness to obtain prior to start Schedule & Compensation Full‑time, Monday–Friday. Salary Range: $119,312.60 – $149,140.20 annually, with bonus eligibility based on individual and company performance. Service Area This position serves patients throughout the South Bay, including: Lawndale, Gardena, Compton, Torrance, Long Beach, Redondo Beach, Hawthorne.
