Position Title Mobile Nurse Practitioner – Southeast Los Angeles Region (Downey, Bellflower, Norwalk, Whittier, Lakewood, Paramount) Job Description Summary With nearly 30 years of experience in providing advanced primary care, CareMore Health delivers exceptional patient experiences in Downey, Bellflower, Norwalk, Whittier, Lakewood, Paramount. Role Overview This is a field-based role requiring travel to various clinical sites and patient locations across our Southeast LA service areas. Responsibilities Provide a range of primary, acute and/or specialty healthcare services for patients in home or virtual settings. Function as the primary care provider in states that do not require collaborative agreements. Order, interpret and evaluate diagnostic tests to identify and assess patients’ clinical problems and health care needs. Provide health instruction and counseling, educating patients on disease prevention and healthy lifestyle choices. Work in collaboration with other physicians, nurses or other healthcare professionals as part of an interdisciplinary care team to treat the needs of patients. Formulate and document the care plan. Prescribe medication or other forms of treatment. Possibly participate in an on-call program, which may include after-hours, weekend and holiday calls and visitation. Service Areas Southeast LA: Paramount, Downey, Norwalk, Huntington Park, Bellflower. Requirements MS in Nursing. Minimum of 1 year of related clinical experience as a nurse practitioner or an equivalent combination of education and experience. Current, active, valid and unrestricted RN license and NP license in Nevada. Satisfactory completion of a tuberculosis test. Compensation $119,313.00 to $178,968.00 The posted compensation range represents the national market average. Compensation for roles located in premium or high-cost geographic markets may fall above this range.
