Registered Nurse

OCCK is looking for a Registered Nurse to provide skilled, person-centered care and help people receive the support they need in their homes and communities.

This position works closely with individuals, families, physicians and other members of the care team to coordinate services, monitor changing needs and support safe, individualized care.

What You’ll Do

• Complete initial and ongoing nursing assessments
• Develop, implement and update individualized plans of care
• Provide skilled nursing care, medications and treatments as ordered
• Educate individuals and families about health needs and plans of care
• Monitor changes in condition and communicate with physicians and care team members
• Coordinate services, equipment and community resources
• Assist with discharge planning and transitions in care
• Serve as Case Manager for an assigned caseload when qualified and assigned
• Complete required clinical documentation
• Supervise and evaluate home health aide services as required
• Participate in the on-call rotation as assigned
• Maintain confidentiality and follow OCCK training, safety and organizational requirements

What We’re Looking For

• Graduate of an accredited school of nursing
• Current Registered Nurse license in Kansas
• Current CPR certification
• Strong assessment, communication, documentation and problem-solving skills
• Valid driver’s license and reliable transportation
• Ability to safely perform the physical duties of the position
• Bachelor’s degree preferred
