Key Responsibilities\n

• Conduct detailed psychosocial assessments through patient interviews, observations, and comprehensive reviews of medical records, to analyze factors affecting patients' overall well-being and recovery.

• Provide tailored counseling and psychological support to patients and their families, addressing issues related to grief, adjustment, and emotional resilience.

• Work collaboratively within the interdisciplinary team to formulate and implement individualized treatment and care plans, ensuring that psychosocial issues are integrated into the medical treatment process.

• Facilitate access to vital community resources, including support groups, vocational rehabilitation services, and housing assistance, to enhance the quality of life for our patients.

• Educate patients and their families on available community resources and post-rehabilitation services, guiding them through available options for ongoing support.
\n
• Participate in discharge planning processes to ensure smooth transitions for patients moving from hospice care to home environments or other care settings.

Patient Population / Client Focus\n

You will work closely with a diverse patient population primarily composed of individuals facing terminal illnesses. This role emphasizes understanding and addressing complex emotional and social challenges that arise during end-of-life care. Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

We offer a competitive annual salary between USD 75,000 – 100,000, commensurate with experience, alongside a sign-on bonus for eligible candidates.
