• #Tier3 Travel Compliance SSN DOB Phone number and email address Accurate certifications based on job description and Nursys report (*must be pulled within the last month) RTO needs to be included in the original submittal. Any RTO outside of the original submittal will not be approved and the traveler will have to discuss with the floor manager once on assignment. Maximum RTO is 10 days Candidates can only be subbed to 1 job 1-2 years of experience is preferred but preference will be listed in the job description Valid State License is not required at submission Candidates within 50 miles of any CHN facility are considered local. Locals are not accepted. 2 references are preferred Candidates must be willing to float within scope of experience within hospital Epic experience is required Will accept travelers with background hits but varies case by case depending on what the hit is Returning travelers are welcome back to the facility without any waiting period in between contracts
