Key Responsibilities\n

In your role as an LICSW, you will take on various responsibilities, including:

• Conducting comprehensive assessments to understand each client's emotional and psychological needs, tailoring interventions accordingly.

• Developing and maintaining accurate clinical documentation, including progress notes and treatment plans, in accordance with organizational policies and legal standards.

• Engaging in regular communication with physicians and other healthcare team members to discuss patient progress, emerging concerns, and potential challenges.

• Working collaboratively in interdisciplinary meetings to create effective treatment plans based on thorough psychosocial assessments.

• Providing crisis intervention and emotional support to patients and families dealing with grief, anxiety, and other emotional challenges associated with serious illness.

• Coordinating care with family members and support systems to foster effective treatment outcomes and ensure client needs are met.

Patient Population / Client Focus\n

As a member of the Directions Counseling Center team, you will focus on clients typically involved in hospice care, including terminally ill patients and their families. Your expertise will play a crucial role in addressing the psychological and emotional impacts of serious health conditions, guiding clients through both anticipatory and active grief processes, and ensuring they are equipped with necessary coping strategies during this significant life transition.

Qualifications\n

To be successful in this role, candidates must meet the following qualifications:

• A Master’s degree in Social Work from an accredited program.

• An active state license as a Licensed Independent Clinical Social Worker (LICSW).
\n
• A minimum of three years of clinical experience in an outpatient setting, with a preference for experience in hospice or palliative care.

• Demonstrated proficiency in computer skills, including familiarity with electronic medical records, word processing, and spreadsheet applications.

• Exceptional organizational skills and the ability to work independently while managing multiple tasks effectively.

Preferred Experience\n

Compensation and Benefits\n
