For many Living Resources employees, their work becomes an experience where they find fulfillment and develop both personally and professionally.The Intake Coordinator manages program admissions for individuals with intellectual and developmental disabilities. The Intake Coordinator will be the point person ensuring all required processes are complete for new individuals to begin receiving supports. This person will work across all programs. The priority will be at the discretion of the Chief Program Officer.Responsibilities include:Manage the admissions and discharge process for individuals receiving supports and services including completing and maintaining all intake/discharge documentation. This will require understanding each program and necessary admission/discharge requirementsKnowledge of Medicaid, Medicare, SNAP and Social Security benefits and applications/certifications for eachPrimary contact for referrals through OPWDD, Care Managers and familiesGather necessary documents for all new admissionsParticipate/coordinate trial visits and pre-placement meetings for all new admissionsEnsure changes to individuals service authorizations and amendments are submitted to OPWDDCollaborate with the finance department and benefit specialists to troubleshoot issues impacting billing and cash flowLiaison with Care Managers and Regional Office for obtaining eligibilityAttending Residential Opportunity Committee meetings in addition to the DRO Day Service Providers meetingsDevelop and maintain an internal system for monitoring enrollment progress. Give reports as necessaryCoordinate all Internal MovementEnsuring all Medicaid and waiver documentation is in place prior to admissionSubmit documentation for enrollment in NetSmart (if required by the program)Establish and maintain professional relationships with key employees involved in Residential Services, Day Habilitation, Community Living as well as, external community professionalsAttend all meetings as assigned by the Associate Executive DirectorThis position requires flexibility as hours may vary.Participate on cross-functional committees when requestedComplete other duties as assignedRequirements:High School Diploma requiredAssociates or Bachelor's Degree PreferredAt least 2 years working with individuals with intellectual disabilitiesValid NYS driver's licenseComplete all state and agency required trainingExcellent verbal, written, interpersonal and communication skillsKnowledge and understanding of local and regional regulatory operationsKnowledge and understanding of OPWDD regulationsUnderstanding and commitment to community-based support for persons with disabilitiesExperience with intake and admissionsExcellent time management skillsProficient in Microsoft Office (Outlook, Word, and Excel)Benefits: We offer great benefits including Paid Training, Health, Dental & Vision Insurance, Flexible Spending Accounts, Payment in Lieu of Health Insurance, Tuition Assistance, and Retirement Programs.
