SIGN ON BONUS- $2,000 Our PurposeInspired Home Health is dedicated to providing compassionate and personalized in-home health care, ensuring the health, comfort, and independence of our clients in the comfort of their own homes.Position SummaryAs a Home Health Physical Therapist serving the West Valley, you deliver skilled in-home therapy that restores mobility, reduces pain, and supports safe independence. This role exists to ensure patients receive evidence-based PT aligned with physician orders and home health standards.Key responsibilities include completing PT evaluations; creating, updating, and carrying out individualized plans of care; providing therapeutic interventions and caregiver education; collaborating with the interdisciplinary team; supervising PTAs as assigned; completing OASIS and all required documentation on time in Homecare Homebase (HCHB); and maintaining compliance with Medicare Conditions of Participation, agency policy, and Arizona regulations.Required qualifications/skills: - Master’s or DPT in Physical Therapy (accredited program)- Active Arizona PT license; CPR certification- Strong OASIS and Medicare knowledge; HCHB proficiency- Valid driver’s license, reliable transportation, and auto insuranceIf you’re ready to provide high-quality home health PT with disciplined documentation and coordination, apply today.How Your Day FlowsYour Monday–Friday routine is organized around scheduled home visits across the West Valley. You’ll start by reviewing new referrals and confirming visit windows with patients and caregivers, then travel between homes with an efficient route. Between appointments, you’ll enter visit notes and OASIS items in HCHB, respond to care-team messages, and connect with the ordering physician when patient status or needs shift. Throughout the day, you’ll stay attentive to home safety considerations and equipment needs. You’ll close out by finalizing documentation within required timeframes and mapping out the next day’s visit plan.Compensation & Coverage- Competitive salary: $100,000–$140,000- $200,000 sign-on bonus- Medical, Dental, and Vision insurance- Life Insurance- Paid Time Off (PTO)
