$2000 SIGN ON BONUSOur PurposeInspired Home Health is dedicated to providing compassionate and personalized in-home health care, ensuring the health, comfort, and independence of our clients in the comfort of their own homes.Position SummaryThe East Valley Physical Therapist Home Health role delivers skilled in-home PT to improve mobility, reduce pain, and help patients remain safely independent. You will perform PT evaluations, create and update individualized plans of care per physician orders, provide therapeutic interventions and caregiver education, and coordinate with the interdisciplinary team. You will complete OASIS and all required documentation on time in Homecare Homebase (HCHB), supervise PTAs as assigned, and follow Medicare Conditions of Participation, agency policies, and Arizona regulations.Required qualifications:- Master’s or DPT in Physical Therapy from an accredited program- Active Arizona PT license; CPR certification- Strong OASIS and Medicare knowledge; HCHB proficiency- Valid driver’s license, reliable transportation, and auto insuranceIf you’re ready to provide high-quality home health care across the East Valley, apply today.Your Typical ScheduleYour Monday–Friday routine centers on a planned route of home visits across the East Valley, with time built in to review new referrals and confirm visit windows with patients and caregivers. Between stops, you’ll document in HCHB, respond to care-team communication, and contact the ordering physician when status changes. Throughout the day, you’ll stay attentive to home safety considerations and equipment needs, then close out by finalizing notes, organizing the next day’s schedule, and ensuring items are completed within required timelines.Total RewardsThis full-time role offers a competitive salary ($100,000–$140,000), plus Medical, Dental, and Vision coverage and Life Insurance. You’ll also receive Paid Time Off to support rest and continuity, along with a $2,000 sign-on bonus.
