Work Environment

This position is entirely onsite, with a focus on home care settings, allowing our therapists to provide treatments in a familiar environment for patients. You will have the flexibility to set your own hours from Monday through Friday, supporting work-life balance while serving your community.

Key Responsibilities

As a Home Care Physical Therapist, you will:

• Evaluate and assess patients' physical conditions and rehabilitation needs.
• Design and implement individualized treatment plans using therapeutic exercises, stretching techniques, manual therapy, and appropriate assistive equipment.
• Continuously monitor patients' progress, making modifications to treatment plans as required.
• Track detailed documentation of treatment sessions, patient responses, and ongoing evaluations, ensuring accuracy and compliance with legal standards.
• Conduct patient and family education sessions regarding therapy exercises, equipment use, and rehabilitation principles to promote independent recovery.
• Collaborate with other healthcare professionals to coordinate comprehensive care, ensuring alignment of therapeutic goals across disciplines.
• Stay up-to-date with advancements in physical therapy practices by attending workshops and participating in professional networking opportunities.

Patient Population / Client Focus

You will be working with a diverse population of patients who may be experiencing challenges due to various conditions such as orthopedic injuries, neurological disorders, or post-surgical recovery phases. The role emphasizes a holistic approach to care, taking into account each patient's unique circumstances, preferences, and goals to foster engagement and ownership in their recovery journey.

Qualifications

The ideal candidate will possess:

• A Doctorate in Physical Therapy from an accredited institution.
• A valid state license in Physical Therapy to practice in Florida.
• Exceptional verbal and written communication skills to facilitate intuitive connection with patients and their families.
• Proficiency in using electronic health record systems and basic computer applications.

Compensation and Benefits
