Case Manager

Community-Rehabilitation-Center

About the Role: We are seeking a dedicated Case Manager to advocate for and empower clients, particularly individuals experiencing or at risk of homelessness with substance use and/or co-occurring disorders. This role involves full care coordination, connecting clients to resources such as SNAP, SSDI, SSI, HUD programs, and community services, while collaborating closely with counselors, local agencies, and program partners. Key Responsibilities: Provide intensive, individualized case management and care coordination. Conduct outreach, presentations, and community events. Support clients in accessing housing, health, and social services. Maintain accurate records and data in compliance with HIPAA. Serve as a liaison for non-English speaking participants. Participate in ongoing program training and development. Qualifications: Bachelor’s degree in behavioral health or related field preferred.

  1. year experience serving homeless or at-risk populations.
  2. years of case management and care coordination experience.
Knowledge of harm reduction, motivational interviewing, and supportive advocacy preferred. Bilingual (English/Spanish) preferred but not required. Skills & Attributes: Strong communication and interpersonal skills. Ability to work independently and as part of a team. Cultural sensitivity and understanding of challenges faced by vulnerable populations. Strong organizational and decision-making abilities.
