Field Case Manager - Elder & Disabled Support (NYC)
NEW YORK CITY HOUSING AUTHORITY
The NEW YORK CITY HOUSING AUTHORITY is looking for Case Managers to provide supportive services to elderly and disabled residents in Manhattan and the Bronx. Candidates should have experience in case management with educational backgrounds ranging from a Bachelor's degree to a High School diploma. Responsibilities include home visits, linking residents to services, and managing wellness programs. Strong communication skills and the ability to speak both English and Spanish are preferred.
