Certified Diabetes Educator I - RD

Job Summary 100% onsite in the Greater Austin, TX area. Outreach clinics located in Hutto, Taylor, Cedar Park, Pflugerville and Buda (clinic frequency TBD). Essential Functions Of The Role Plans, directs, and provides individualized self‑management education, training and help to patients and families in a culturally‑competent manner across the lifespan. Develops and conducts educational programs and in‑service training programs. Serves as a resource to staff and community as a diabetes care and education specialist. Optimizes care delivery, patient and provider experience, quality, safety, and costs. Applies current principles of teaching and learning and/or behavior change to facilitate self‑management skills. Tailors interventions to individual self‑management needs. Communicates information to patients and healthcare team in a timely manner. Serves as a role model, resource, and mentor to other team members. Participates in quality improvement activities to help process improvement and clinic/facility/system goals. Participates in professional development activities to maintain and improve required qualifications; obtain 15 diabetes‑related Continuing Education hours annually.

KEY SUCCESS FACTORS

Advanced knowledge of pre‑diabetes and diabetes care modalities, therapies, terminology and equipment. Familiarity with pre‑diabetes and diabetes pathophysiology, epidemiology, and evidence‑based clinical practice. Familiarity with ADA Education Recognition requirements or National Standards for Diabetes Self‑Management Education and Support, if in an ADA Recognized program. Clear verbal and written communication skills. Effectively uses a variety of teaching methods. Knowledge of the geographic area, the community and its resources. Cultural proficiency related to health practices. Committed, dependable, adaptable, original, resourceful with an excellent desire to help the community. Positive interaction with patients as needed. General computer skills, including but not limited to: using required software applications, data entry, information security, electronic medical documentation, handheld scanning and email. Certified Diabetes Educator certification strongly preferred within 12 months of hire. Knowledge of insulin pumps and Continuous Glucose Monitoring (CGM) devices strongly preferred. CERTIFICATION/LICENSE/REGISTRATION
