This position provides advanced, comprehensive professional nursing services in the care and treatment of patients within a community health care center and at satellite service provision sites. Must have a current registered nurse license in the state of Florida. Must have a bachelor’s degree or one year of experience as a registered nurse. One or more years of experience in family planning services, sexually transmitted infections, HIV, and/or women’s health. One or more years of experience with utilizing an electronic health record. Works under the supervision of the Assistant Executive Community Health Nursing Director and Registered Nursing Consultant, in conjunction with the assigned Advanced Practice Registered Nurse. Coordinates case management activities for patients who have chronic diseases and are living with HIV and family planning patients, patient’s with sexually transmitted infections. Complete 500/501 HIV Counseling and Testing class within 3 months of hire. Complete certification for STD Clinician within six months of hire. Organizes and provides services for patients with exemplary customer service. Records accurately all nursing care provided in EHR. Administers medications and vaccines with care and accuracy utilizing two patient identifiers. Records the medications and vaccines in the appropriate systems (PFS and FLSHOTS) prior to administering and with accuracy. Accurately maintains inventory and monitors vaccines and medical supplies/equipment. Works in collaboration with internal and external partners. Ensures adherence to nursing protocols, as well as DOH policies and procedures. Ensures adherence to billing procedures. Accurately applies nursing principles and techniques in the direct care and treatment of patients. Correctly makes assessments and document’s patient health status. Assess, treats, evaluates, and provides health education and follow up with appropriate cultural competence. Positively assists medical provider in clinic session. Interprets and implements physician orders and recommendations to patients appropriately. Completes and follows up on referrals as appropriate and accurately documents all clinically relevant information. Provides exemplary customer service during telephone and walk-in triage as necessary in consultation with the medical provider. Properly obtains vital signs and medical history; and performs simple laboratory procedures (blood draws, finger stick, heel stick). Willingly and proactively assists in maintaining inventory of medical supplies and medications; and prepares examination/treatment rooms and equipment. Promotes cooperation within the agency and externally by maintaining positive relationships with Management Team, Medical Providers, Nurses, Health Support Technicians, Human Service Counselors, Senior Clerks and other staff. Develops and maintains an interdisciplinary team approach to patient care and case management. May participate in quality improvement activities, including the peer review process. Participates in outreach activities for people living with HIV as needed. Assists the STD Field Operations Team (DIS) to provide treatment for patients with positive labs for STDs and/or contacts to such under the DOT and/or Nurse Issuance protocols Attends clinical staff meetings, in-services, seminars and trainings for self-improvement, increased competency, and retention of skills. Participates in the orientation and staff development programs for persons assisting with client care. May oversee the work activities of ancillary personnel. Ensures timely completion of all mandatory training. Ensures accurate and timely submission of employee leave and attendance records, Employee Activity Records (EAR's), and travel vouchers. Travel is performed in relation to above duties. Access to a personal vehicle and a current Driver’s License are essential to the performance of the duties described for this position. Qualifications: Minimum: Active and in good standing license as a Registered Nurse in accordance with Florida Statute 464 and Florida Administrative Code 210-8.22 OR 8.27 Must have a valid driver's license. Must have experience in HIV and/or sexually transmitted infections. Must have experience drawing blood.
