Registered Nurse (RN) – Home Health

Schedule: Monday–Friday, 8:00 AM–5:00 PM
Hours: 40 hours guaranteed
On-Call: Every 3 weeks
License: KY RN or Compact RN
Productivity: 30 points/week
Mileage: $0.43/mile
Counties: Boyd, Carter, Greenup, Elliott & Lawrence
Not expected to cover all counties in one day.

Job Overview

Seeking an experienced Home Health RN to assess high-risk patients, develop individualized care plans, and coordinate services to support positive outcomes. The RN will collaborate with physicians, social workers, and other healthcare professionals to ensure appropriate and cost-effective care.

Key Responsibilities

Perform comprehensive patient assessments and develop individualized care plans.
Coordinate care with providers, social workers, discharge planners, and community resources.
Manage care transitions and connect patients with medical and non-medical support.
Monitor patient outcomes and adjust care plans as needed.
Identify opportunities for health promotion and disease prevention.
Advocate for patients while maintaining privacy and confidentiality.
Maintain accurate clinical and care management documentation.

Requirements

Active KY or Compact RN License
Associate or Bachelor's degree in Nursing or related field
1+ year of Home Health experience required
HCHB experience required
Strong clinical assessment and critical-thinking skills
Case Management certification
Ability to meet 30 productivity points/week
