Cadence is seeking a Clinical Navigator (Medical Assistant) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. You will conduct patient outreach, coaching, and care coordination to close gaps in care, while using Cadence platform, Google Workspace, and Slack to collaborate with the clinical team. The role is full-time and remote, based in the United States, with standard hours 8:00 AM–5:00 PM PT or 9:00 AM–6:00 PM MT/PT. #J-18808-Ljbffr Cadence
