Key Responsibilities

• Conduct comprehensive evaluations, including the review of medical history and performance of specialized assessments, to inform treatment planning.
• Engage patients and their families by clearly explaining evaluation findings and integrating their input into personalized goal-setting for treatment.
• Implement evidence-based treatment protocols tailored to individual patient presentations and monitor progress using standardized tools and methodologies.
• Collaborate with a team of healthcare professionals, recognizing and documenting significant changes in patients' conditions that may necessitate modifications in their care plans.
• Ensure effective discharge planning and follow-up care by providing thorough documentation, including initial evaluations, progress notes, and summaries that reflect each patient's journey.
• Maintain an organized schedule and demonstrate effective time-management skills to meet productivity expectations and patient demand.
• Participate in hospital and community marketing initiatives to create awareness of available rehabilitation services and improve access to care.
• Actively contribute to achieving departmental goals by engaging in strategic planning and adhering to compliance with regulations and quality standards.
• Pursue ongoing professional development opportunities and serve as a mentor to junior therapists and students.

Patient Population / Client Focus

At Grandview Rehabilitation Center, our primary focus is on patients who require orthopedic rehabilitation. This includes individuals recovering from orthopedic surgeries, sports injuries, and musculoskeletal conditions. Qualifications

• Bachelor's degree in Physical Therapy is required; a Doctorate in Physical Therapy is preferred.
