Occupational Therapist (OT) - Mobile Outpatient

Role Overview This full-time Occupational Therapist provides outpatient rehabilitation services primarily in in-home settings for older adults, including private residences and independent living facilities. The role involves evaluating and treating patients to improve functional performance, promoting safety, and adapting care plans to meet individual needs. Key Responsibilities Conduct assessments, develop and implement personalized treatment plans, and document progress. Supervise support personnel, students, and assistants as applicable. Collaborate with healthcare team members and refer patients to other disciplines when needed. Order supplies, maintain equipment, and ensure safety standards. Design and train patients in the use of adaptive devices and assistive technology. Evaluate and modify treatment approaches based on patient progress and needs. Promote high-quality, compassionate patient care and foster community reintegration. Qualifications & Skills Bachelor’s degree in an accredited occupational therapy program. Initial certification by the National Board for Certification in Occupational Therapy. Valid, unencumbered occupational therapy license in the practice state. Reliable transportation and a valid driver’s license.
