Step into a pivotal role providing in-depth home health risk assessments for Medicare Advantage members residing in Drew County, AR. Key Responsibilities:

• Perform annual home health risk assessments, including both standard health reviews and post-discharge visits, averaging 45 minutes per appointment
• Assess patients prior and current medical histories, ongoing conditions, and medication regimens
• Deliver education to patients regarding their health conditions and prescribed medications
• Detect gaps in care and suggest actionable recommendations to mitigate potential risks
• Note: This position does not require treatment planning or prescribing medications

Qualifications:

• Active Nurse Practitioner License in Arkansas (AR)
• Current certification in CPR (BLS)
• National board certification through AANP or ANCC
• Minimum of 6 months experience working as a Nurse Practitioner preferred
• Master of Science in Nursing (MSN) from an accredited institution

Compensation for this role is $80-$90 per evaluation, complemented by a travel stipend for mileage and additional compensation for administrative time.
