Job Title & Role Description We are seeking a Care Platform Product Manager responsible for driving the strategy, design, and delivery of technology solutions that support clinical care operations across the organization’s care platform. This role focuses primarily on solutions built in Salesforce Health Cloud , with occasional collaboration on integrations or workflows involving our Electronic Health Record (Athena). The Product Manager partners closely with clinical leaders, care teams, and operational stakeholders to deeply understand pain points, translate those needs into well‑defined product requirements, and guide solutions from concept through delivery. This individual works closely with the Product Development team—including Product Analysts and Salesforce Developers—to prioritize work, define acceptance criteria, manage scope, and ensure high‑quality releases. The Care Platform Product Manager will own the product roadmap for clinical platform capabilities, balancing new requests against strategic priorities while maintaining visibility into delivery progress and outcomes. The role requires strong stakeholder engagement, comfort navigating ambiguity, and the ability to establish structure and product discipline in a fast‑evolving healthcare environment. Skills Required Experience managing healthcare or clinical technology products Strong familiarity with care management workflows and clinical operations Experience working with Salesforce Health Cloud or comparable care management platforms Ability to translate complex operational needs into clear product requirements and user stories Experience writing detailed acceptance criteria and working closely with engineering teams Strong stakeholder management and cross‑functional communication skills Experience managing product roadmaps and prioritization frameworks Ability to track product performance using metrics and outcomes Experience managing scope, timelines, and delivery expectations Strong presentation and communication skills for executive and stakeholder updates Experience working in agile or iterative product development environments Ability to manage multiple projects simultaneously in a dynamic environment Key Behaviors Develop a deep understanding of clinical workflows and care team pain points to inform product strategy Serve as the primary product owner for Care Platform capabilities, primarily within Salesforce Health Cloud Partner with clinical stakeholders to define requirements, document workflows, and develop clear acceptance criteria Translate stakeholder needs into actionable user stories and prioritized backlog items for the development team Collaborate closely with Product Analysts and Salesforce Developers to design and deliver scalable solutions Own and maintain the Care Platform product roadmap, balancing strategic initiatives with new requests Manage stakeholder expectations and proactively communicate project status, timelines, risks, and tradeoffs Establish clear processes for intake, prioritization, and scope management to reduce ambiguity and scope creep Track key metrics and outcomes to evaluate the effectiveness of delivered features and platform improvements Prepare presentations and status updates for leadership and operational stakeholders Coordinate testing, validation, and stakeholder approval for new features and enhancements Ensure delivered solutions align with both operational needs and broader organizational strategy Competencies Product Strategy and Roadmapping Healthcare Workflow Expertise Stakeholder Influence and Relationship Management Analytical Thinking and Problem Solving Communication and Executive Presentation Organizational and Project Management Skills Adaptability in Ambiguous Environments Process Design and Operational Structure Cross‑Functional Collaboration Outcome and Metrics Orientation Upward Health is committed to attracting, retaining, and maximizing the performance of a diverse and inclusive workforce.
