Patient Care Navigator
SEARHC
SEARHC is looking for a Primary Care Coordinator to coordinate comprehensive patient care, liaise with the care team, manage referrals, and support plan-of-care communications. The role emphasizes patient-centered service and timely follow-up within the Southeast Alaska health system. Qualified candidates will have an associate degree or equivalent experience, strong communication skills, and experience with electronic health records; a Bachelor’s degree is acceptable in place of experience. #J-18808-Ljbffr SEARHC
