- Coordinate and oversee patient care plans to ensure optimal health outcomes and efficient use of healthcare resources.
- Assess patients' medical conditions, social determinants of health, and care needs to develop individualized care plans.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure continuity of care.
- Coordinate insurance authorizations, appeals, and approvals for necessary medical services and treatments.
- AK Registered Nurse license
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
- Basic Life Support (BLS) certification
- Case Management Certification (CCM) or Accredited Case Manager (ACM) certification preferred
- Pay: $2,443.27 - $2,518.84 / week
- Location: Juneau, AK
- Assignment Length: 15
- Shift: Day 5x8 - up to 40.00 hours per week
- Setting: Hospital
- Workplace: On-site
