Social Worker (Program Coordinator)-HCHV

Summary: This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Applicants must meet specific eligibility requirements per VHA policy and submit an EDRP application within four months of appointment. Program award amounts (up to $200,000) and eligibility periods (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the application. Former EDRP participants are ineligible to apply. Qualifications United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA policy. Education: Must hold a master’s degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Doctoral degrees are not substitutes for the master’s degree. Verification of the degree may be made by visiting . Licensure: Persons hired in the GS‑0185 series must be licensed or certified by a state to independently practice social work at the master’s degree level. English Language Proficiency: Must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Grade Determinations: In addition to the basic requirements, the following criteria must be met when determining the grade of candidates: Social Worker (Program Coordinator) – GS‑12: Requires one year of experience equivalent to the GS‑11 grade level that demonstrates advanced practice skills and judgment. Candidate may have certification or other post‑master’s degree training from a nationally recognized professional organization or university that includes a defined curriculum or internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinators must be licensed or certified at the Advanced Practice Level and capable of providing supervision for licensure. Demonstrated Knowledge, Skills, and Abilities (KSA): Must demonstrate knowledge of program coordination and administration, including consultation, negotiation, and monitoring; ability to write policies, procedures, and/or practice guidelines; supervision of multidisciplinary staff; and organization and prioritization skills. Assignments: For all assignments above, the full performance level must include significant scope, complexity, and variety at least 25% of the time. Program coordinators at the GS‑12 grade level are administratively responsible for clinical programs providing treatment to Veterans in major specialty areas (e.g., spinal cord injury, hospice, suicide prevention, etc.). Program coordination responsibilities include developing policies and procedures, preparing reports, managing budgets, providing training and coaching, conducting audits, and evaluating program performance. Travel: Up to 75% of work may include travel. Driver License: Must possess a current and active California driver license. Duties Identify and engage Veterans experiencing homelessness or marginally housed and who underutilize VA services. Provide a range of social work services, including assessment and referrals, clinical treatment, short‑term case management, advocacy, service coordination, and linkage with VA and community resources. Collaborate with the CBOC interdisciplinary team and community providers to deliver services. Participate in outreach activities that promote information on services and benefits available at SFVAHCS. Work confidently with city and county agencies to increase resources for on‑the‑street or marginally housed Veterans. Facilitate VA involvement in the Coordinated Entry System (CES) and assist SFVAHCS in meeting federal CES guidelines. Coordinate efforts with local planning and support initiatives from VA, HUD, and U.S. Interagency Council on Homelessness (USICH). Develop a comprehensive psycho/social assessment of each Veteran in collaboration with the interdisciplinary team. Document assessments with appropriate Returning to Work (ROI) notes and complete HOMES assessments as required. Reassess needs and revise the initial intake plan as needed. Prepare an Individualized Service Plan (ISP) with measurable goals, referral plans, and specific history components. Provide case management services tailored to individual Veteran needs and ensure follow‑through. Advocate for Veterans, encouraging self‑advocacy and empowerment. Provide education on VA and community resources, entitlements, and advance directives. Offer ongoing supportive counseling to assist Veterans with homelessness and barriers to housing. Lead interdisciplinary treatment groups such as Sobriety Support, Harm Reduction, and Advance Directive Education. Duty Location: 401 Third Street – San Francisco, CA 94107. Work Schedule: Monday‑Friday 8:00 am to 4:30 pm (or as determined by the hiring manager/supervisor). Travel Demand: Up to 75% of work may involve travel.
