FLU VACCINE IS REQUIRED at UMMC - only medical exemption is allowed and requires documented evidence from a health care provider of a past severe reaction to the vaccine

FINGERPRINT REQUIREMENT - Remote process must allow 10 days, In Person can be done at Jackson or Grenada campus the week prior to start

THIS HIRING MANAGER IS LOOKING FOR HIGHLY EXPERIENCED CANDIDATES WITH ACUTE INPATIENT CASE MANAGEMENT AND DISCHARGE PLANNING EXPERIENCE
Training on unit will be minimal - candidates will need to be able to pick up quickly and work efficiently.


**REQUIRED:
- ACTIVE License - LMSW or LCSW AND ACTIVE MS State License
- Minimum 2 years of Acute Inpatient Case Management experience that includes at least 1 year in a LARGE Hospital (500+ beds, LEVEL 1/ACADEMIC Medical Center) (this does NOT include telephonic, utilization management, or therapy experience),
- Previous travel in a level 1 facility
- Strong computer skills, Epic EMR experience,
- Acute care/ED discharge planning experience, Recent (within the last two years) acute inpatient case management experience.


Shifts & Scheduling
36/48 rotating schedule: 7a-7p every Wednesday & Thursday and every other Friday, Saturday, Sunday
Weekend Requirements None
Holiday Requirements Rotated as needed
Case load/# patients per day/consults per day 15-20


INTERVIEW PROCESS - leaders interview via webex (on screen interview) Please provide SPECIFIC times candidate is available to be on camera for a 20-30 min interview for the 72 business hours following submission.

UMMC has contracts with 5 local facilities with 10 beds reserved at each facility to assist with the complex placements, however specific criteria must still be met. Criteria has a limitation on financial assistance for only those uninsured or underinsured.
UMMC is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options.

Scrub Color/Dress Code Any color scrubs or business professional attire
RT for DC Planning? Will consult with RT as need
Pharmacy for DC Planning? Will consult with Pharmacy as needed
Department Specific Orientation 2 weeks (will include general facility orientation) Will work alongside CM team so will never be working alone.
