Intake Coordinator

Job TitleResponsibilitiesFollow up on referrals received from marketing representatives and referral sources.Manage the intake process for patients seeking home care services.Coordinate and follow up on patient approvals with insurance companies, and other vendors.Communicate regularly with patients, family members, referral sources, physicians, and outside vendors.Maintain accurate and up-to-date patient records and intake documentation.Ensure all required documentation is collected and submitted in a timely manner.Monitor referral status and provide updates to patients and referral sources.Assist with staffing and case coordination when needed.Work closely with intake, coordination, and management teams to ensure timely service initiation.Meet departmental goals and performance expectations.QualificationsStrong organizational and multitasking abilities.Must have at least 1 year of home care experienceSpanish speaking is a plusExcellent verbal and written communication skills.Ability to work effectively in a fast-paced environment.Strong attention to detail and follow-through.Ability to prioritize tasks and manage competing deadlines.Team-player mentality with a positive attitude.Ability to handle pressure and adapt to changing priorities.Previous home care, healthcare, intake, coordination, customer service etc
