Behavioral Health Care ManagerThe Behavioral Health Care Manager (BHCM) provides high-quality, member-centered behavioral health care management services to Titanium Healthcare's Enhanced Care Management (ECM) population. The BHCM is responsible for engaging members with complex behavioral health and psychosocial needs, coordinating care across medical and behavioral health providers, connecting members to community resources, and promoting improved health outcomes through evidence-based interventions. The BHCM serves as the behavioral health expert on the interdisciplinary care team, providing clinical consultation, supporting care planning, facilitating care transitions, and ensuring members receive comprehensive, integrated care.This position is hybrid. Work from home and go into the field (San Bernardino & Riverside Counties) as needed. Travel is required to conduct in-person member visits within designated areas. You will have full control over your schedule when meeting members. You are eligible for mileage reimbursement for the use of your vehicle for business-related travel. Standard business hours are Monday through Friday from 8:00 am to 5:00 pm.Member Care ManagementManage ECM members with behavioral health needs, primarily Tier 1 and Tier 2 membersConduct comprehensive biopsychosocial assessments both telephonically and in personDevelop therapeutic relationships that promote engagement, self-management, and improved health outcomesUtilize evidence-based interventions including Motivational Interviewing, Behavioral Activation, solution-focused techniques, and brief therapeutic interventionsProvide crisis assessment and facilitate warm handoffs to crisis response resources when appropriateCoordinate behavioral health, medical, and social services to ensure integrated, whole-person careSupport members during transitions of care, including hospital admissions and dischargesConduct face-to-face visits based on member risk stratification and health plan requirementsMaintain ongoing engagement through telephonic outreach, home visits, and community-based encountersCare Coordination & Clinical ResponsibilitiesCollaborate with primary care providers, specialists, behavioral health providers, hospitals, health plans, and community organizationsParticipate in Interdisciplinary Care Team (ICT) meetings, Systematic Case Reviews (SCRs), and case conferencesCollaborate in the development and ongoing maintenance of member-centered shared care plansCoordinate referrals for behavioral health treatment, substance use treatment, housing, transportation, food insecurity, and other social determinants of healthMonitor member progress toward behavioral health and wellness goalsTrack required assessments, screenings, authorizations, and documentation within the care management platformEnsure timely documentation that meets Titanium Healthcare, MCP, and DHCS requirementsEducation & ConsultationEducate members and families regarding behavioral health conditions, treatment options, medications, and healthy coping strategiesProvide consultation to interdisciplinary team members regarding behavioral health conditions, crisis management, and community resourcesAssist with staff education regarding behavioral health best practices and evidence-based care approachesPromote integrated behavioral and physical healthcare throughout the organizationQuality & ComplianceMaintain compliance with Titanium Healthcare policies, DHCS regulations, ECM requirements, and Managed Care Plan (MCP) standardsSupport quality improvement initiatives through accurate documentation and timely completion of required member activitiesParticipate in audits, quality reviews, and performance improvement initiativesComplete all assigned documentation within established timelinesPerform other duties and projects as assignedWho You AreStrong knowledge of behavioral health assessment, diagnosis, and treatment planningUnderstanding of Serious Mental Illness (SMI), Substance Use Disorders (SUD), trauma-informed care, and recovery-oriented practicesKnowledge of Enhanced Care Management (ECM), CalAIM, and population health principlesExperience utilizing Motivational Interviewing and other evidence-based engagement techniquesExcellent crisis assessment and de-escalation skillsStrong care coordination and interdisciplinary collaboration skillsAbility to navigate complex healthcare and community resource systemsExcellent documentation and organizational skills.Ability to prioritize workload and manage multiple competing prioritiesProficiency electronic health record systemsSensory RequirementsFluent in English (written and verbal)Ability to communicate clearly in-person, by phone, and electronicallyAdequate hearing and vision (with corrective devices if necessary) to conduct assessments and documentationAbility to identify problems and use logic and related information to develop and implement solutionsExcellent organizational skills and attention to detailCommitment to maintaining patient confidentiality and adhering to ethical standards in healthcare practicePhysical ActivityAbility to lift, carry, push, or pull up to 20–25 pounds (e.g., laptop bag, forms, mobile equipment)Ability to climb stairs or navigate uneven terrain in community and home environmentsAbility to bend, reach, and conduct in-person visits in non-traditional environmentsMust be able to remain in a stationary positionMust be able to move around the office and/or travel throughout communityAbility to operate a vehicle and travel to meet with members around the community;
