Bluebird Homecare is looking for an experienced, organized, and motivated Director of Coordination to lead our coordination team and help deliver exceptional care to our patients across New York.

This is a leadership role for someone who thrives in a fast-paced environment, enjoys solving problems, and knows how to build and manage a high-performing team. If you're passionate about providing outstanding customer service while keeping operations running smoothly, we'd love to meet you.

Responsibilities

• Lead and supervise our team of home care coordinators.
• Ensure patients receive the highest level of service and support.
• Manage caregiver scheduling and staffing to maximize coverage.
• Oversee patient care coordination.
• Monitor team performance, productivity, and quality standards.
• Train, mentor, and develop coordination staff.
• Handle escalated patient and caregiver concerns with professionalism and compassion.
• Work closely with clinical staff, HR, intake, and management to ensure seamless operations.
• Develop and improve workflows to increase efficiency and patient satisfaction.
• Track key performance metrics and identify opportunities for improvement.

Qualifications

• Previous management experience in a New York home care agency required.
• Strong understanding of HHAeXchange and home care operations.
• Excellent leadership and team-building skills.
• Outstanding communication and customer service abilities.
• Highly organized with exceptional attention to detail.
• Strong problem-solving and decision-making skills.
• Ability to multitask and prioritize in a fast-paced environment.
• Experience working with Medicaid, MLTCs, and home care compliance is preferred.
• Proficiency with Microsoft Office and other office software.
