Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Outpatient Clinic team. The ideal candidate will have a passion for patient care and possess the ability to perform in a fast-paced clinical environment. The LPN will be responsible for providing high-quality nursing care to patients while collaborating with healthcare teams to ensure optimal patient outcomes.

Responsibilities

• Deliver direct patient care, including medication administration and assessments.
• Collaborate effectively with multidisciplinary team members to create and implement patient care plans.
• Monitor patient conditions and provide timely updates to healthcare team.
• Educate patients and their families on health management and wellness strategies.
• Maintain accurate and timely documentation of patient progress and care interventions.

Required Experience / Certifications / Licensure

• Current New York State LPN license.
• BLS Certification required.
