Acute Care Hospital Case Manager

Assignment Duration

This is a 13-week contract position.

Schedule

Day shift, 5 days a week (08:00 - 16:30) with weekend rotation as needed.

Job Description

The Acute Care Hospital Case Manager will be responsible for Care Coordination and Discharge Planning within an acute care setting. The ideal candidate will have hands-on experience in managing patient care processes, ensuring a seamless transition from hospital to home or other facilities.

Responsibilities

• Perform care coordination and discharge planning for patients in an acute care environment.
• Assess patient needs and develop individualized treatment plans.
• Participate in interdisciplinary meetings to discuss patient care and treatment options.
• Arrange for post-discharge services, including DME, SNF, and transportation.
• Ensure compliance with CMS and InterQual Criteria for admissions and discharges.
• Maintain accurate and thorough documentation of patient interactions and care plans.

Required Experience / Certifications / Licensure

• 2 years of experience as a Case Manager in an acute care hospital setting is required.
• CA State License in good standing is mandatory; pending licenses will be accepted but must be obtained before the start date.
• Experience with the Epic charting system is preferred.
• Knowledge of drug/alcohol abuse treatments and mental health issues is a plus.
