Title
Acute Care Hospital Case Manager

Schedule
- Shift: Orientation 40 hours
- Weekend requirement: Weekend rotation as needed
- Holiday requirement: Must commit to working 3 of 5 major holidays

Requirements
- California RN license required (pending accepted only if fully licensed by start date)
- Minimum 2 years of acute care hospital case management experience
- Ability to independently arrange DME, SNF, and ambulance transport
- Experience with HMOs/IPAs, family meetings, treatment plan participation, and assessing patients for discharge planning
- Proficient in using Epic for computerized documentation

Roles & Responsibilities
- Coordinate patient care and facilitate discharge planning.
- Assess patients for their discharge needs and create appropriate treatment plans.
- Conduct family meetings to discuss care plans and address concerns.
- Utilize InterQual criteria for admission and care coordination.
- Arrange necessary services, including DME and transport to SNF when required.

Compliance / Notes
- Pre-start modules: Non-billable, average 16 hours
- Dress code: Scrubs or business attire
- Must be available for required holidays as noted.
