Key Responsibilities

• Conduct comprehensive evaluations to assess patient needs, integrating assessment data with patient history to develop personalized treatment plans.
• Effectively communicate assessment results and treatment strategies to patients and their families, ensuring they are active participants in the goal-setting process for rehabilitation.
• Implement evidence-based treatment protocols tailored to each patient's individual needs, adjusting techniques as required by patient condition and healthcare advancements.
• Collaborate with interdisciplinary teams, recognizing and documenting any significant changes in a patient's health that necessitate updates to the treatment plan.
• Manage discharge planning with precision, ensuring comprehensive follow-up care is in place and that all documentation is completed promptly and accurately, including evaluation notes, progress reports, and discharge plans.
• Maintain effective time management to meet patient scheduling and productivity expectations while actively promoting our clinic's accessibility and quality of care.
• Contribute to departmental objectives, ensuring compliance with all governmental and accreditation standards, while participating in the development of quality improvement initiatives.
• Pursue ongoing professional education and training, serving as a mentor and clinical instructor for students and junior therapists.
• Enhance patient experience through flexibility and adaptability in service delivery, actively working to remove barriers to patient access and engaging with community outreach initiatives.

Patient Population / Client Focus

Qualifications
