Responsibilities

• Deliver coordinated nursing care for patients in a home hospice setting according to established standards and the nursing process.
• Assess patient data, identify care needs, and develop and implement care plans; adjust plans as needed.
• Collaborate with the patient, family, and multidisciplinary team to ensure safe, compassionate, and goal-directed care.
• Educate patients and families, provide health information, and participate in discharge planning to support continuity of care.
• Direct and coordinate duties of the nursing team, delegating tasks appropriately while maintaining safety and quality standards.
• Document care delivery and outcomes, and contribute to ongoing evaluation of patient progress.

Requirements

• Active Registered Nurse license in California (state of practice)
• CPR or Basic Life Support (BLS) certification required
• Advanced Cardiovascular Life Support (ACLS) certification required
• Hospital Fire and Life Safety (HLFS) certification required
• Bachelor’s Degree in Nursing (BSN) preferred
• At least two years of recent nursing experience in the field; acute care experience preferred
• Ability to work in a home healthcare – hospice setting
