Pay Range: $24 to $27 per hour (DOE/DOQ) Work Schedule: 40 hours/week, Monday to Friday 403(b) Retirement Plan Life Insurance Long Term Disability Wellness and Employee Assistance Programs LifeFlight Membership JOB PURPOSE Performs duties of Care Coordination that enhance treatment engagement and retention, reduce service duplication, strengthen relationships between clients and their care providers, align and maximize use of available resources, and facilitate monitoring follow-up and continuity of care. Additional services include providing coaching on skill development, running groups, and providing care management services. Assures that services are congruent with age, strengths, supports and cultural framework of the individual being served. ESSENTIAL DUTIES AND RESPONSIBILITIES Collaborate with members of the outpatient clinical teams to develop Individual Service Plans that address psychosocial needs and guide the focus of treatment. Develop care coordination protocols, sharing information between entities with appropriate consents for release of information. Coordinate care between entities to access high-quality physical health (both acute and chronic) and behavioral health care, social services, housing, educational systems, and employment opportunities as necessary to facilitate wellness and recovery of the whole person. Obtain appointments with other entities and follow up to confirm appointments were kept, ascertain individuals' preferences and needs for care, and document in clinical records that can be shared with consented entities. Develop a crisis plan with each client to ascertain advance directives in event of psychiatric or substance abuse crisis. Coordinate care that enhances treatment, engagement, and retention, reduces service duplication, strengthens relationships between clinics and care providers, aligns and maximizes use of available resources, facilitates monitoring, follow up, and continuity of care. Coordinates transitional care between hospitals, detention centers, and mental health institutions to return to community. Engage clients across the lifespan in treatment, focusing on skill development in areas such as problem solving, goal setting, symptom management, communication, social skills, and how to access community resources. Provides ongoing assessment of a client's cognitive, emotional, physical, social, and vocational strengths and needs. Addresses activities that impact behavioral health, including sleep, exercise, media, and nutrition. Assists in the development of natural support systems for clients. Maintains familiarity with and handles client information in accordance with Federal Regulations (42 CFR, Part 2), the Revised Code of Washington (RCW 71.05.390 and RCW 71.24), and other applicable laws pertaining to confidentiality of client and staff information. Maintains physical security of confidential materials and assigned Agency property. SECONDARY DUTIES AND RESPONSIBILITIES Performs relevant clinical and administrative tasks according to agency policies. Demonstrates proficiency in utilization of the Agency's electronic medical record system. Participate in team meetings and program development. Provides information and education to community members. Maintains high standards of professional conduct in interaction with clients, staff, and other community members. Maintains professional competence through a program of professional development and in-service training. Demonstrates flexibility in adapting to changing work demands. Performs other duties as assigned or requested. PERFORMANCE EXPECTATIONS Maintains accurate, timely chart information in compliance with quality assurance guidelines established by funding sources, State and PBH. Provides direct service to or on behalf of PBH clients for at least half of the hours worked annually. Meet agency expectations for documentation of services. Coordinate and prioritize work assignments so that tasks are completed on time and in a quality manner. REQUIRED EDUCATION, LICENSE(S), CERTIFICATION, AND EXPERIENCE Education: Bachelor's degree in social services, psychology, or related field Licensure: Agency Affiliated Counselor Registration (assistance provided) Experience: Two years' experience working with clients with emotional or behavioral challenges Other: Must possess and maintain a valid driver's license and a satisfactory and insurable driving record to be able to drive company vehicles Additional requirements: Must be able to pass a pre-employment drug test and background check. Must be able to provide vaccination records for MMR, Hep B, Tdap and recent flu shot. KNOWLEDGE, SKILLS, AND ABILITIES Ability to multitask and effectively manage a caseload of individuals with diverse needs. Knowledge of how mental health symptoms may present differently in clients across the lifespan. Ability to educate clients about signs, symptoms, and treatment of their diagnosis. Understanding of current knowledge on recovery. Knowledge of social service delivery systems. Ability to coordinate care with multiple service providers. Ability to work independently. Ability to communicate effectively both orally and in writing. Capability to use sound judgment in problem solving and clinical intervention. Practice respectful communication with clients to enhance rapport and positive outcomes. Ability to plan and prioritize work and meet the Agency's performance, quality assurance, and productivity standards. Ability to communicate effectively with our mostly English-speaking client population. NATURE AND SCOPE Physical Demands Moderate physical effort occasionally lifting up to ten pounds. Must be able to operate a motor vehicle. Must be able to tolerate sitting in a vehicle, office or community setting for the required number of hours in a workday. Cognitive Skills Advanced verbal and written communication skills. Must be able to enter data into a computerized electronic medical record. May be exposed to infections and contagious diseases. Occasionally exposed to patients exhibiting assaultive behaviors Able to visit with clients in office and community settings. Working Demands Contact with patients under a wide variety of circumstances. Subject to varying and unpredictable situations Handles emergency or crisis situations. Available to work occasionally on Saturdays as needed. Able to work from the Sequim office as needed. Ensuring compliance with agency and funding standard for documentation and delivery of services.
