This unique setting allows for personalized, one-on-one interactions with clients in their own surroundings, facilitating a more comfortable and effective rehabilitation experience. You will utilize advanced therapeutic equipment and modalities, adapting treatment plans to each patient's specific needs. Collaborating with a multidisciplinary team, you will contribute to a supportive atmosphere that prioritizes patient-centered care.Key ResponsibilitiesConduct thorough patient evaluations to assess mobility, strength, and functional capabilities to tailor effective, evidence-based treatment plans.Implement individualized therapy sessions focusing on rehabilitation, pain management, and recovery of physical function at the patient's home.Collaborate with other healthcare providers, including MDs and OTs, to create a comprehensive care pathway for each patient.Educate patients and their families on therapeutic exercises, injury prevention techniques, and at-home strategies to enhance recovery.Maintain meticulous and timely documentation of patient progress and treatment outcomes using our integrated electronic medical records (EMR) system.Participate in regular clinical rounds and case conferences to review patient cases and discuss strategies for improved care.Patient Population / Client FocusThis role will primarily focus on adult patients experiencing various musculoskeletal and orthopedic challenges. Expect to work with individuals post-surgery, those managing chronic pain, and clients recovering from sports injuries.
