Location: SNF facility, CA 90804
Start Date: ASAP
Duration: 13-week contract
Hours: 36 hours/week
Pay: $85–$87/hour
Setting: Skilled Nursing Facility (SNF)
Assignment Type: Travel
Position Summary & Responsibilities:
Evaluate and treat SNF residents requiring physical therapy services
Develop and implement individualized treatment plans
Improve patient mobility, strength, balance, and functional independence
Document evaluations, progress notes, and discharge planning accurately
Collaborate with interdisciplinary healthcare team to support patient care goals
Educate patients and caregivers on therapy plans and safety strategies
