Job Title: Registered Nurse Profession: Nursing Specialty: Case Manager Duration: 12 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 3 years of related experience required. License: Valid state nursing license required. Certifications: BLS certification required. Must-Have: - Ability to assess, evaluate, and drive discharge plans. - Coordinate services to the patient's appropriate level of care. - Ensure that treatment plans are initiated, evaluated, and re-evaluated daily. - Follow patient progress and identify barriers to discharge. - Monitor length of stay and follow regulatory guidelines. Description: This position involves the ability to assess, evaluate, and implement discharge plans effectively. You will coordinate services to ensure patients receive the appropriate level of care. Daily evaluation and adjustment of treatment plans are essential to meet patient needs. Monitoring patient progress is critical, as is identifying and addressing any barriers to discharge. Knowledge of regulatory guidelines is necessary, specifically regarding patient choice and notifications. This role requires strong communication and organizational skills to manage a diverse patient population. Interdisciplinary collaboration and care coordination are key components of this position. You will be expected to manage a case load with a focus on quality patient outcomes. Flexibility may be required to float within facility as needed. This position does not require weekend or holiday shifts. All candidates must meet compliance and onboarding requirements prior to starting.
