Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.MINIMUM REQUIRED QUALIFICATIONS –TX/compact RN licenseBLS1+ year case management experience in an acute hospital settingAcute hospital experience during the past year with no more than a 90-day breakPREFERRED QUALIFICATIONS –CCMMeditech & Careport experienceLENGTH OF ASSIGNMENT – 13 weeksSHIFT / HOURS PER WEEK – 8A-4:30PSYSTEMS – Meditech, CareportSTART DATE – 9/21/2026
