Mobile PhysioCare is a clinician-owned outpatient practice led by Dr. Vaishali Parmar, PT, DPT, MBA, CEEAA. We bring one-on-one physical, occupational, and speech therapy into homes, assisted living, group homes, and day habilitation programs across Bergen, Hudson, Passaic, and Middlesex Counties, New Jersey.

Caseload mix: NJ DDD / delayed developmental disability, Medicare Part B, commercial PPO, and cash-pay visits. This is outpatient therapy in the home or day hab — not a Medicare home-health agency. No OASIS. No homebound requirement.

There is no OASIS documentation and no homebound requirement. You treat like a skilled outpatient clinician, in the place the patient actually lives or spends the day.

Responsibilities

• Provide skilled Physical Therapist Assistant services in a day habilitation program

• Treat adults with developmental and related disabilities in a structured day setting

• Document outpatient-style notes the same day — no OASIS

• Collaborate with program staff, families, and supervising therapists

• Keep treatments functional: transfers, mobility, ADLs, communication, and community skills used in the program

Requirements

• Current New Jersey PTA license

• Graduate of a CAPTE-accredited PTA program

• Valid driver’s license, reliable vehicle, and auto insurance

• Comfort treating adults with developmental disabilities in a group program

Preferred

• Geriatric, neurologic, or developmental-disability experience

• Spanish or Korean

• Ability to cover more than one listed town

Pay and benefits (required on Indeed in New Jersey)

• $40-$42 per hour in the day habilitation program

• W-2 employment

• Predictable on-site schedule

• Liability insurance

• Mileage if you float between sites

• Medical, dental, and vision when scheduled 30+ hours W-2

Pay: W-2 is biweekly (work two weeks, paid the following Friday). New W-2 per diem clinicians can take a $500–$1,000 visit advance after the first completed work week; it is deducted from paycheck #2. 1099 contractors are paid weekly after signed notes.
